School Learning and Support Team model is a whole-school planning and support mechanism. Its purpose is to address the learning needs of students, the professional needs of teachers and to make recommendations regarding allocation of available and appropriate resources.
All students enrolled at SCHS receive educational case management by their allocated teacher. At times students will be referred to the LST or principal due to additional/complex support needs requiring coordinated education/health action planning with families and census schools.
The LST works closely with the Clinical Nurse Specialist | Kids GPS Care Coordination for students that are in hospital and are not allocated to a team such as Kids Cancer Centre (KCC) or Brain Injury and Rehabilitation Team (BIRT).
The School holds an Oncology Learning & Support (LST) Team meeting with Ronald McDonald Learning Program (RMLP), Back On Track (BOT) and any relevant health professionals. The meeting is held at SCHS on Monday's from 12pm – 1pm. All oncology students enrolled in SCHS are discussed.
The purpose of the LST is to support the education program for students enrolled in our school by:
- increasing understanding of the educational program that SCHS has in place for each student
- addressing student educational support needs and identify students that need additional support
- identifying shared students with other service providers such as RMLP, BOT and Child Life Therapy and share relevant information across services
- coordinating actions needed for each student and the communication to census school
- educational transition planning
